Geriatric Assessment Center

The DMH Geriatric Assessment Center has a team of professionals to help families identify concerns about caring for aging family members and future living arrangements. Held every other Friday by appointment at the DMH Rehabilitation Center, this outpatient assessment service helps families identify health issues and discuss long range planning options. The appointment, which is covered by Medicare, usually does not require a physician referral.

A medical doctor handles a verbal assessment and performs a complete physical, including lab work or x-ray services, if needed. Then, patients are evaluated by a neuro psychologist. A gerontological certified nurse is always present during the assessments, which can last between one and one-and-a-half hours. Additional testing may be suggested and an appointment scheduled.
